One of the most frustrating situations imaginable takes place when a spouse, adult child or friend knows that their senior loved one is in need of care and supportive services, yet doesn’t know where to go for help.
That’s where the Bethesda Care Management program can play an important role in making sure your older adult receives the care and services they need.
Comprised of a Certified Geriatric Care Manager, Care Coordinator and Personal Services Assistants, this specialized team, with more than 60 years of senior services experience, will provide an assessment and develop a plan customized to each individual based on need. This plan will be created in conjunction with a number of different sources, including family, physicians, hospital social workers and many other healthcare professionals.
Geriatric Care Manager
As part of the assessment, our Geriatric Care Manager will:
- Evaluate the senior’s needs
- Determine possible eligibility for assistance programs
- Provide guidance in planning for current healthcare needs, as well as those that may be required in the future
Other services that are provided include:
- discussing Advance Directives, Power of Attorney, Long-Term Care policies and other important documents
- facilitating family meetings
Looking for an answer to a question involving senior care and services? Look no further than Bethesda’s Care Coordinator. Even if your question addresses a question or service outside of Bethesda, the Care Coordinator will research the issue and provide you with the information you need to find the answer.
Personal Services Assistants
The Personal Services Assistants are available to make life easier for your loved one – and for you! Not only will they serve as an escort to a doctor appointment, but they can take notes and provide you with a report. They can help coordinate medical appointments, including transportation. They can help with a number of functions within the home, such as paying bills, correspondence, errands and general home organization.
