
Care Management
One of the most frustrating situations imaginable takes place when a spouse, adult child or friend knows that their senior loved one is in need of care and supportive services, yet doesn’t know where to go for help.
That’s where the Bethesda Care Management program can play an important role in making sure your older adult receives the care and services they need.
Comprised of experienced Care Coordinators and Personal Services Assistants, this specialized team will provide an assessment and develop a plan customized to each individual based on need. This plan will be created in conjunction with a number of different sources, including family, physicians, hospital social workers and many other healthcare professionals.
What Our Care Management Team Can Do
As part of the assessment, our Care Management team will:
- Evaluate the senior’s needs
- Develop customized plans of care
- Determine possible eligibility for assistance programs
- Provide guidance in planning for current healthcare needs, as well as those that may be required in the future
- Discuss Advance Directives, Power of Attorney, Long-Term Care policies and other important documents
- Facilitate family meetings
Care Coordinators
Looking for an answer to a question involving senior care and services? Look no further than one of Bethesda’s Care Coordinators. Even if your question addresses an issue or service outside of Bethesda scope of offerings, the Bethesda Care Coordinators will research the issue and provide you with the information you need to find the answer.
Personal Services Assistants
The Personal Services Assistants are available to make life easier for your loved one – and for you! Not only can they serve as an escort to a doctor appointment, but they can take notes and provide you with a report. They can help coordinate medical appointments, including transportation. They can help with a number of functions within the home, such as paying bills, correspondence, errands and general home organization.

Respite Care Grant Program
For the third year in a row, Bethesda has received the Phyllis and Milton Berg Family Respite Care Grant from the Alzheimer’s Foundation of America (AFA).
Funding from this grant will allow Bethesda to offer much-needed financial support to families who are caring for a loved one with Alzheimer’s disease and related illnesses, and would benefit from one or more of our Respite Care services.
