Serve as a liaison between Bethesda lab and pharmacy & the Director of Nursing and staff nurses.  Impromptu rounds in the facility for keeping work areas clean and clear of obstacles, CNAs and CNs performing duties in a timely fashion and to assess resident care.  Weekend on call duties include:  staffing, incident reporting, and trouble shooting issues that come up.  Occasionally when needed, attends care plan meetings with staff and family members.  Completes assigned section of MDS in a timely manner.

Monitors employees currently on Worker’s Compensation therapies or PT/OT/ST therapies and types up the notes and informs the Director of Nursing.

Performs minor maintenance on the vehicle (refilling of windshield washer fluid, checking oil, tires, coolant levels, keeping the vehicle clean, etc.) to ensure the vehicle operates smoothly and efficiently. Returns vehicle to Bethesda Dilworth at the end of the day with a full tank of gas.

Communicates professionally and appropriately with residents, members of other departments, and the Administrator.  Communicates concerns of residents to the Administrator or the Activity Director.

When the driver is not transporting residents on an outing or maintaining the bus, he/she is responsible for assisting the Activities department as needed.  This includes being informed of the daily activity calendar, assisting residents to planned activities, helping lead activities, and performing appropriate documentation. 

Perform one-on-one room visits with residents as directed by the Activities Supervisor or Director. Document outcome of visits as appropriate.

The MDS Coordinator is responsible for all long-term care resident Minimum Data Sheets and resident MDS chart audits.  Ensures that the facility is in compliance with the Division of Aging regulations. 

Responsible for completing all long-term care MDS, admission, significant change, discharge, re-entry tracking forms quarterly and annual assessments.  Responsible for the completion of all Resident Assessment Protocol and entering all necessary information into the computer system. Maintains records of all  MDS/RAPS for a 15-month period.

 Ensures all departments involved in the Resident Assessment Instrument (RAI) process complete their MDS sections/RAPs and Care Plans in according to the time line set by the MDS Coordinator to stay within state guidelines.  Prepares and distributes time lines and calendars to all departments involved in the RAI.

 Attends the care plan meeting with all the other departments and participates in the care plan process.    

 Transmits MDS assessments successfully to the Division of Aging (minimally every thirty days, preferably weekly).

 Notifies supervisors and charge nurses of any concerns or problems in a timely fashion.

 The MDS Coordinator must conduct MDS chart audits on all admissions and/or re-admissions for accuracy and competency.
